take down sleeve layout

I just orderd a 3-rivers small dia. t/d sleeve.  My question is do i make the center of the four inch sleeve the center of my bow, or will it be offset?

That's a good one.  I've done only two of these and I've never considered offsetting the handle.  I've only considered making sure the handle lines up with the axis of the limb.
It depends on the bow and the design.  If the handle is not centered the bow might feel like its rolling when you draw the bow.
Normally I would center the handle, begin the tiller, find out which side the string wanted to go (selfbow) and make that side my shelf side.

I done one. I just fit it to center and then layed out my bow. I still ended up with 1" above center and 3 below. That's how I always lay out my self bows with the arrow pass 1 to 1.5" above center.

The main thing is getting it centered to your back on both pieces

Per Diamondback's sleeve-sticking problem, what I found was that the inner sleeve might buldge a little after gluing to the limb and therefore fits too tight in the outer sleeve.  Or, it will just be too tight for comfort from the beginning.  
Either way, wax the inner sleeve to see if it will then slide in and out.  If not, pull it out and use sandpaper where the wax shows rubbing, then polish with steelwool, then rewax and try again.  Repeat as necessary.  
If the sleeve is assembled and will not pull out, put the sleeve/bow in a vise and gently squeeze the sleeve from front to back while pulling out the inner sleeve.  Increase vise pressure slightly as needed to free the inner sleeve.  
The wax should come with the handles as it can make an impossible sleeve combo assemble perfectly fine.
