When you guys buy or get wood for your laminations, do you get air dried wood, kiln dried or something other?  And if you cut your own wood to make laminations, is there a certain moisture content you get them to before cutting and what moisture content do you use when glueing up?

If I use Kiln dried, I let it set in my shop for a week or so to settle into my climate.
If I use air dried, I make sure it has been air drying for 2-3 yrs Minimum. Sometimes I go by the rule 1yr dry time for 1" of thickness, and then I go by how the wood acts while working it.
I don't use a moisture meter and I try to keep my shop at about 30-40% RH
I have never had a problem when I follow this method, and I don't push it.
