zingers high and left?

What's going on?  I've been realy focusing on my form lately and it has tighned up my shooting.  The problem is that I get one that flyes high and left every now and then.  Many times it's my first arrow.

Am I not getting enough back tension?  Maybe I should film myself and let you fellas look at my form.

Is it the same arrow all the time?  I had a similar problem shooting compound years back.

If its the same arrow, the solution I found is a simple one.

Rotate your nock and use a different cock feather.  This solved my problem.  I hope it helps you.

Number your arrows and see if it's always the same one.  The other common causes are;
1) bad release-plunking string-too much pressure on third finger...the MOST common.
2)grip - lower grip of pad on riser means higher landing.
3) Arrow a bit stiff and hitting shelf (a fletch tick is all it takes) - 1/8" difference in anchor (closer to target or collapsing on release) will cause the arrow to stiffen and impact high-left.
